To pick up your win-loss statement, go to Ohiya Casino’s Guest Services and show your Player’s Club Card and valid photo I.D. Ohiya Casino will print your win-loss statement, W2-G, and a 1099 summary for you.
Once we receive your completed form, requests will be processed and mailed back to you. Please allow up to one week for Ohiya Casino to process your request. Illegible or incomplete requests may not be processed.
